Published adult memberships in Duval County range from $65 a month at The Well Healthcare in Orange Park's neighbor Clay County to $149 at Elite Health and NOLA Care, with most between $79 and $125. Children are $10 to $99 depending on the practice. Two Duval practices do not publish a price.
Which Jacksonville DPC practices accept Medicare?
Balanced Physician Care accepts Original Medicare as primary but not Medicare Advantage. Green Leaf Direct Care has a senior tier for Medicare-age patients. NOLA Care lists Medicare among accepted payers. Coastal Medical and Wellness does not currently see Medicare patients. Always confirm the practice's Medicare opt-out status before enrolling.
Can I use my HSA to pay for a Jacksonville DPC membership?
Since January 1, 2026, a DPC membership of $150 a month or less for an individual, or $300 for a family, no longer disqualifies you from contributing to an HSA, and the fee itself is a qualified expense. Every Duval practice with a published adult price is at or under $150. Bundled medications or non-routine labs can affect eligibility, so ask the practice what the fee includes.
